Clarivoy needs to obtain 10 years of Historical Service data (Closed ROs) for your dealership, plus ongoing daily data for new and updated ROs.

We have a relationship established with Tekion, where Tekion will directly provide us with the requested data once your dealership authorizes the data sharing.

Allow Tekion to Send Your Service Data to Clarivoy

1. Please print or download ALL THREE OF these forms below (use the print or download buttons in the upper right corner of the form):
   1. Historical Closed ROs Data Authorization
   2. Daily Closed ROs Data Authorization
   3. Daily Service Appointments Data Authorization
2. On each form, complete the 2 sections with a green box around them (Dealer Information and Dealer Authorization).  
3. Tekion requires a physical signature on each form.
4. Complete one form per store, as required by Tekion.  
       ◦ If you have one store, you will complete three forms (one for Historical ROs, one for Daily ROs, one for Daily Appointments).
       ◦ If you have two stores, you will complete six forms (two for Historical ROs and two for Daily ROs, two for Daily Appointments).
       ◦ If you have three stores, you will complete nine forms, etc.
5. Please return the completed forms to Clarivoy, not Tekion.  Please upload the form inside your Onboarding portal in the Tekion Service data tasks, or send them to datateam@clarivoy.com.
        We need to add Secure FTP credentials to your completed form before sending to Tekion.
